Disruptive Mood Dysregulation Disorder (DMDD) is a relatively new diagnosis that was added to the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) in 2013. It is characterized by severe and recurrent temper outbursts that are disproportionate to the situation and inconsistent with the developmental level of the individual. DMDD is commonly diagnosed in children and adolescents, but it can also be diagnosed in adults.
The symptoms of DMDD are different from those of other mood disorders such as bipolar disorder and major depressive disorder. Unlike bipolar disorder, individuals with DMDD do not experience periods of mania or hypomania. Similarly, individuals with major depressive disorder do not exhibit the chronic irritability and temper outbursts that are characteristic of DMDD.
The exact cause of DMDD is not yet known, but research suggests that it may be a combination of genetic, environmental, and neurobiological factors. There is evidence that DMDD runs in families, suggesting that genetics may play a role in the development of the disorder. Environmental factors such as stress, trauma, and parenting style may also contribute to the development of DMDD. Neurobiological factors such as abnormalities in the prefrontal cortex, amygdala, and striatum have also been associated with DMDD.
The diagnosis of DMDD is typically made by a mental health professional such as a psychiatrist, psychologist, or licensed clinical social worker. A thorough evaluation is necessary to rule out other disorders that may have similar symptoms such as bipolar disorder, major depressive disorder, oppositional defiant disorder, and ADHD.
Treatment for DMDD typically involves a combination of medication and psychotherapy. Medications such as atypical antipsychotics, mood stabilizers, and antidepressants may be used to manage symptoms such as irritability, impulsivity, and aggression. Psychotherapy, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), can help individuals with DMDD learn skills such as emotion regulation, distress tolerance, and interpersonal effectiveness.
Individuals with DMDD and their families need to have a strong support system. This can include mental health professionals, family members, friends, and support groups. Education about the disorder can also help understand and manage the symptoms of DMDD.
