The development and course of many diseases, including acne inversa, are strongly influenced by diet, among other factors. In acne inversa, nutrition plays a crucial role as it influences various mechanisms that contribute to the pathogenesis of this skin disease.
Acne inversa is characterized by the formation of inflamed abscesses, fistulas and nodules in the affected skin areas. The pathogenesis of acne inversa is influenced by several factors, including "hyperkeratosis," in which hair follicles become clogged with too much keratin produced, leading to inflammation. This process can be influenced by a variety of factors including diet. Avoiding triggering factors, called triggers, also plays a major role in the healing process.
An unhealthy diet rich in saturated fats, sugars and refined carbohydrates can increase inflammation in the body. This, in turn, can increase the production of pro-inflammatory molecules such as tumor necrosis factor alpha (TNF-alpha). TNF-alpha is a pro-inflammatory cytokine that promotes inflammation in the body and may be involved in the development of acne inversa.
In addition, an unbalanced diet can lead to hormonal changes that affect sebum production in the hair follicles. Some foods can affect hormonal balance and stimulate increased production of certain hormones such as androgens (male sex hormones), which in turn promote sebum production. Increased sebum production promotes the formation of comedones (blackheads) and thus also contributes to the development of acne inversa.

Let's take a closer look at how foods can influence these relationships in acne inversa.
Certain foods can promote inflammation in the body, which can negatively affect the course of acne inversa. These include foods high in saturated fats, trans fats, refined sugars and processed carbohydrates. Dairy products are also suspected to have a negative impact on the course of their disease in many AI patients. These foods can lead to an increase in pro-inflammatory markers in the body and thus increase inflammation in the affected areas of the skin.
1. saturated fats:
Foods such as red meat, butter, cream and high-fat cheeses contain high amounts of saturated fats. These fats can have a pro-inflammatory effect as they can increase the production of pro-inflammatory messengers in the body, such as pro-inflammatory cytokines. This can lead to worsening of inflammation in acne inversa.
2. trans fats:
Trans fats are found in many processed foods such as fried foods, snacks, baked goods, and fast foods. They are produced by a process of partial hydrogenation of vegetable oils to extend their shelf life. Trans fats have pro-inflammatory properties because, among other things, they can increase the activity of certain pro-inflammatory genes in the body. This can increase the inflammatory reactions in acne inversa.
3. refined sugar:
Foods high in refined sugar, such as soft drinks, candy, and sugary baked goods, can quickly raise blood sugar levels. High blood sugar levels lead to increased insulin production, which in turn can stimulate pro-inflammatory processes in the body. This can increase inflammation in acne inversa and negatively affect the course of the disease.
4. Processed carbohydrates:
Foods such as white bread, white rice, and pasta consist of processed carbohydrates that are quickly converted to sugar. These foods have a high glycemic index, which means they raise blood sugar levels quickly. A high glycemic index can promote pro-inflammatory processes in the body, thus increasing inflammation in acne inversa.
